EMPOWERMENT APPROACH TO CONTROL MALARIA IN NORTH MALUKU PROVINCE, INDONESIA


Malaria as a one of the tropical and sub tropical disease have a considerable impact in determining future development, especially areas because malaria affects brain growth, fetal growth and development, the rate of school attendance of children up to the productivity.
Control malaria in Northern Maluku empowerment approach. This is done because the complexity of malaria so illness and thus require involvement of all parties such as government, private and community, measurable and sustainable. There are 2 things empowerment conduct in North Maluku interpreted as “ Let them solve their own problems, we just need to open their feeling and insight”.
Institusional empowerment efforts have been started since 2003 through advocacy to the governor of North Maluku. Then number 4 Governor issued instructions in 2003 regarding the establishment of “malaria center” and also instructed the district for establish a similar effort. Malaria Center in Northern Maluku  were as “coordinative institution in charge of disease control malaria is the head of governor at both regional and provincial regent/major at the district level”.
In line with efforts to dissemination and socialization to the district governments, in 2007, community empowerment initiated by using the methode of participatory learning and action, especially in southern Halmahera District as a pilot project. TOT PLA at the district level consisting of stakeholders. Then they become facilitators for the participants of the elements of the village. The village facilitators and than make the same thing at the village level respectively.
The training of the PLA, the public is given an understanding of malaria, malaria transmission, malaria symptoms by methods of “body mapping”, the danger of malaria, where the breeding site by village mapping methode, a review of the brood (transect walk) and making society work plan.
The output obtained is in the form of community work plan that includes efforts to control malaria resourched community. With the patern of this empowerment, Halmahera malaria cases in southern, North Maluku Province, Indonesia decreased by 57,47 % in 2009 compared with 2006 (before PLA).
Besides an impact on decreasing norbidity and mortality due to malaria, this pattern also have an impact on other environment-linked disease such as dengue.
The concept of empowerment that is applied in Northern Maluku refers to the theory of “Stair – case phenomenon”. This theory saw the pattern of life, culture, needs and community needs ( need & demand) vary from one region to other region. There are people who “poor internal” empowerment efforts need to be done and there are also people who poor externally, which only needs a boost.
Now, efforts have been replicated in the Morotai island district n Tikep municipality, North Maluku Province with the concept of PLA training integrated with maternal and child health and immunization programs.
